Use this planner to turn a broad coaching need into a clear request. The better you describe your goal, timeline, blockers, and preferred support style, the easier it is to find a coach who fits.

Fill this out before requesting a coach

You do not need perfect answers. Write short notes for each section. Then copy the strongest points into your coach request so the match starts with useful context.

1. Name the change you want

Write the goal in plain language. Avoid vague phrases like be better or get unstuck unless you explain what that means in daily life.

I want to change careers without losing momentum.
I want better structure and accountability around my business goals.
I want to rebuild confidence after a major transition.

2. Define the 90-day outcome

A good coaching request explains what progress should look like soon, not someday.

I have narrowed my career path to two realistic options.
I have a weekly system I actually follow.
I communicate boundaries without overexplaining or avoiding conflict.

3. List the main blockers

Name the things that keep getting in the way. This helps match you with the right coaching style.

I overthink and do not take action quickly enough.
I start strong but lose consistency after two weeks.
I need outside accountability because I work alone.

4. Choose your support style

Different coaches work differently. Be honest about what helps you follow through.

Direct feedback and structured assignments.
Reflective conversation and mindset work.
Practical planning, weekly checkpoints, and accountability.
